How do they work?

The possessive changes based on the thing owned, not the person who owns it. We usually also use the article.

Attention: with family in the singular we often don't use the article: my mother, your father, his brother.

Adjective or pronoun?

When it accompanies a noun it is an adjective. When it replaces a noun it is a pronoun.

Questo è il mio libro.

Questo libro è il mio.
